1. Acknowledgment of activities and risks

I understand that participation at Miracle's PlayLand LLC may include crawling, walking, running, climbing, sliding, jumping, playing on soft-play equipment, interacting with other children, and other recreational activities. These activities involve inherent and other risks, including falls, collisions, slips, trips, cuts, bruises, sprains, fractures, illness, allergic reactions, and other injuries. I voluntarily allow the child(ren) identified above to participate and knowingly assume these risks.

2. Parent/guardian responsibility and supervision

Miracle's PlayLand LLC is not a daycare or childcare provider. Unless expressly stated for a specific supervised program, I or another responsible adult designated by me must remain on the premises and actively supervise my child(ren). I am responsible for ensuring they follow posted rules, staff instructions, age restrictions, and equipment guidelines.

3. Release and waiver of liability

To the fullest extent permitted by Georgia law, I, on behalf of myself and the minor child(ren) identified above, release and hold harmless Miracle's PlayLand LLC and its owners, members, managers, employees, agents, representatives, landlords, and affiliated parties from claims arising from the ordinary risks associated with participation, including claims based upon ordinary negligence to the extent such claims may lawfully be released. This does not waive claims that cannot legally be waived under Georgia law.

4. Medical authorization

If an injury or medical emergency occurs and I cannot immediately provide instructions, I authorize personnel to contact emergency medical services and take reasonable steps to obtain emergency assistance. I accept financial responsibility for treatment or transportation, except to the extent another party is legally responsible.

5. Health, safety, and facility rules

I will not bring a child who has symptoms of a potentially contagious illness or otherwise presents a health risk. I agree to follow rules concerning socks, footwear, clothing, food, beverages, equipment use, age restrictions, and designated play areas. Staff may stop participation or require a guest to leave when conduct presents a safety risk or violates facility rules.

6. Personal property and property damage

I am responsible for belongings brought into the facility. Miracle's PlayLand LLC is not responsible for lost, stolen, misplaced, or damaged property except where liability cannot legally be excluded. I accept responsibility for damage caused by intentional misconduct or unreasonable misuse by myself or children under my supervision, subject to applicable law.

7. Governing law and acknowledgment

This agreement is governed by Georgia law. If a provision is invalid or unenforceable, the remaining provisions are intended to remain effective. By signing, I confirm that I am the parent or legal guardian, or have legal authority to sign; I have read and understand this agreement; and I voluntarily permit the listed child(ren) to participate.
